There's no need to post multiple times about the same issue.
First make sure that your APN settings are configured correctly.
Text "APN" to 50048 and follow the instructions here:
Then follow the instructions here to set up the hotspot

iPhones don’t support configuration messages so you have to do it manually.
Settings > Mobile Service/Mobile Data > Mobile Data Network
Mobile Data > APN > 48months.liffeytelecom.com
MMS > APN > 48months.liffeytelecom.com
MMS > MMSC > http://mmc1/servlets/mms
MMS > MMS Proxy > 10.1.11.19:8080
Personal Hotspot > APN > 48months.liffeytelecom.com
Restart your phone and it should be working then. The ‘Personal Hotspot’ setting will show up in settings

The problem is that Apple add another hoop to jump through and it isn't immediately obvious how to get past this.
In this case the personal hotspot option doesn't even appear and the iPhone tells the user to contact their network provider.
