What message are you seeing on the phone now? Roughly how many times have you attempted to enter the PUK?
What error do you get when entering the PUK?

Are you sure that it's looking for the PUK and not the PIN/PIN1?
Are you sure that you're using the correct PUK?
The one from your online account?
