If you can't get it to your old number/SIM because it is disabled then you'll need to contact 48 support for assistance:

What do you mean unlocking code for your 48 SIM?
If you mean the PIN (PIN1) or PUK then see here:

Did you register your SIM?
Is your phone unlocked for use on any network?
Have you scanned for networks and manually connected to 48 (or O2 if 48 is not listed)?
Did you enable data roaming (required even if not using data)?
What does it say when you log into your online 48 account?
