Somebody asked about configuring their personal hotspot earlier today and I replied but, for some reason, the thread then disappeared completely.
In case it helps here are the steps needed to configure/enable one's personal hotspot:
(1) Ensure that mobile data is working
Make sure that you have an active membership plan with a non-zero mobile data balance and then configure the APN settings so that mobile data works for browsing and connected apps:
If you are on Android you can text "APN" to 50048 and follow the instructions here to do this:

If you are using an iPhone/iOS device and have problems configuring the personal hotspot due to the convoluted way that Apple do things, then see this key post - in particular the section titled "A note about personal hotspot setup on iPhones":
