Hi Team,
My phone has been stolen and SIM card lost and I tried to reorder the SIM again. The SIM came and we tried to activate it. There was no other option to block as we could see that the option was graded out (screenshot below). Please suggest what needs to be done on this.

Your screenshot isn't displaying correctly.
This link explains how to obtain and activate a replacement SIM and optionally block the lost/stolen SIM:
Isn't it the case that the option to block the lost/stolen SIM appears when ordering a replacement SIM and not when activating the replacement?
Whether or not you blocked the old SIM when ordering a replacement, activating the replacement will certainly block the lost/stolen SIM and render it useless.
