If you mean a new SIM then you register/activate it here:

Bear in mind that this will NOT activate on an existing account/number if you have one. If you want to do that then you need to log a support request to get 48 to assist.

If you mean a replacement SIM then you do that through your online account:
http://48months.ie/help/losthandset
If you mean that your existing SIM has somehow gone faulty or become deactivated then you'll need to log a support request to get help from 48 support.
