PIN not accepted when opening phone

I turned on my phone screen and was prompted to enter my PIN, which doesn't usually happen. When I entered it I was then prompted to enter a 4-8 digit PIN, and when I did so nothing happened. I have not been given the option to enter my PUK at any point.

Did something change on the phone before this happened? 

What is the exact prompt? 

Please describe in detail the exact steps taken from turning on the phone to "nothing happening".

E.g. turn on, prompt for SIM PIN (?), enter SIM PIN, another prompt for a 4-8 code appears (?), what exactly is the prompt?, what do you enter here, what do you mean by "nothing happens", etc.

What make and model of phone is it? 

What happens if you take the 48 SIM out and reboot the phone?

Sounds more like a phone than a 48/SIM problem? 

You might need to contact the seller or manufacturer for advice?
