Where do I enter the code to activate SIM?

I am unable to activate it because my SIM arrived in the mail. I already have an email and password to pay and log in, but I cannot find a place on the website to enter the CODE: I have already followed the instructions in my plan, but I do not see a place to enter the CODE.
