Isn't the error message pretty clear? 

You obviously entered the wrong details when requesting your number port so you need to double check them with your old provider and try again with the correct details?

Very often this happens when people try to enter an account number when their old account was a registered pay as you go contract with no account number.

You should contact your old provider for advice.
