Unrequested switching to voice mail issue

Over the past month, my iPhone is switching to voicemail on an ongoing basis and I am missing many calls as a result.
What is the solution to cancelling this?

Voicemail forwarding is enabled by default on 48.

How do I switch voicemail on/off?

You can switch off your voicemail by dialing 172 or enter the above short codes for the preference you’ve set up. If you’re not sure of which option you originally switched on, then enter #004# to check the status.
