Forgot my unlock code

I entered my unlock code wrong 3 times and now it wants me to contact my carrier and get a PUK code? .. Who do I contact? I was with meteor for years but I got a 48 sim last year and unlocked it unofficially.. Not sure what to do

The PIN/PUK have nothing to do with whether or not a phone is locked to a specific network.
