This info is not available as a matter of course.

You'd need to contact 48 support and see if they can provide it on request:

Your phone and/or specific apps are other ways to track usage locally instead of having to contact support.

You'll need to post a bit more info.

E.g. when did you start experiencing problems?

What problems exactly - e.g. low signal bars, complete outages etc.?

Where are you located?

Did anything change on your phone before you started having these problems?

Have you tried switching off, taking the SIM out, gently cleaning the contacts, reinserting the SIM, switching on and manually scanning for and connecting to 48 again?
